Home Hospice Care

Hospice care at home allows patients to remain in the comfort of their familiar surroundings, surrounded by loved ones. Skilled nurses, social workers, home health aides, spiritual counselors, and volunteers all work together under the direction of the patient’s physician to ensure every moment is met with comfort and dignity.

Support is available 24 hours a day, 7 days a week, with all medications, supplies, and equipment related to the hospice diagnosis provided. Home hospice care allows families to focus on meaningful moments, knowing their loved one is cared for with compassion and expertise.

Palliative and Supportive Care

While often confused with hospice care, Palliative Medicine is available at any age and any stage of a serious medical illness. It can be provided alongside current treatments and medications, helping patients manage symptoms, reduce discomfort, and improve overall quality of life throughout the course of their illness. Palliative Medicine is focused on living well—it is not limited to end-of-life care.

Nursing Facilities

Grace Healthcare Services partners closely with nursing facility staff to deliver coordinated end-of-life care that enhances residents’ comfort and quality of life. Our collaborative approach in pain management and symptom control helps prevent unnecessary hospitalizations and improves overall satisfaction for residents and families alike.

Our trained clinical staff work hand-in-hand with facility teams to integrate the hospice benefit into the resident’s care plan, ensuring seamless communication and compassionate support. We attend care conferences, offer 24/7 clinical guidance, and remain dedicated to honoring the trust placed in us by our facility partners and their residents.

Assisted Living Residences

At assisted living communities, our hospice team provides compassionate, individualized care that allows residents to age in place comfortably and with dignity. We collaborate with facility staff to manage pain, oversee medications, and ensure care aligns with both community standards and state regulations.
By working together, we ensure residents receive the highest level of comfort and personal attention while supporting families with ongoing communication, education, and peace of mind. Hospice at assisted living residences is designed to maintain the resident’s independence and enhance quality of life through every stage.

Bereavement Support

At Grace Healthcare Services, our care extends beyond the patient’s life. We are committed to supporting families through the grieving process with compassion and understanding. Bereavement services are available for at least 13 months following a loved one’s passing, offering ongoing comfort and guidance from our social workers and spiritual counselors.

Families receive regular mailings and educational materials on coping with loss, as well as phone calls and personal visits as needed. Our goal is to walk beside each family, offering emotional and spiritual support as they heal and find peace in the months ahead.
